SCR: Professionals lost sight of children

An SCR has criticised the professionals involved in a case where a three-month-old baby died in Doncaster for focusing too much on the mother. The young mother had complex needs, including mental health concerns, and the review found that agencies lost sight of the children involved as they focused on the mother’s needs. The baby was shaken by the mother’s partner, who has been convicted of manslaughter. The review found a “’worrying” lack of information about the mother’s partner, who was acting as a parent and carer.
