Social work failures highlighted in murder of 4-year-old by father

The failure of social workers and children’s guardian to challenge an independent social worker report was the key error in the case of Alexa-Marie Quinn, who was beaten to death by her father. The serious case review found that social workers in Bedford were confused about status and function of the court-ordered report, and felt unable to delay the case when new concerns emerged.
